Further development of the 3LGM2 and specification of a reference model for the domain layer of the 3LGM2 of a hospital

Background:

The main enterprise function of a Hospital Information System is to provide an optimum patient care, including the administration and an economic business management of the enterprise. Hospitals are service enterprises which a high heterogeneous enterprise structure. The main function of the strategic management of a hospital is planning, directing and monitoring of the information system. By describing or modelling of the hospital information system (HIS) it is possible to recognise the heterogeneous enterprise structures. To assist the strategic management of a hospital different meta models of information systems are applied. For the static view of hospital information system the UML-based three-level graph- based meta model (3LGM2) has been developed.

 

 

Goals:

Goal of this project is to operate and assist to develop the 3LGM2-method and the respective tool to be an useful tool for the strategic management. As a result of the tight cooperation with the university of Leipzig, the Information Technology for Healthcare (ITH) and the Tyrolean Provincial Hospitals (including the University Hospital and another four hospitals) there is the possibility to influence into the development the know-how, the experiences of the different professional groups. We are also working on reference models for the domain layer. So the availability of reference models would provide a high valuable contribution to keep the costs for the creation of models of hospital information systems within bounds as they only need to be adapted to the respective information system. This may further lead to a more unique terminology and description of the elements of HIS models. Another goal of this project is to define quality parameters to assess a HIS. Additional quality parameters directly based on a 3LGM2 description would further improve strategic IT-management.

 

 

Methods:

As a result of the tight cooperation with the university of Leipzig, the Information Technology for Healthcare (ITH) and the Tyrolean Provincial Hospitals (including the University Hospital and another four hospitals) there is the possibility to influence into the development the know-how, the experiences of the different professional groups. This reference model may be based on the requirements index for information processing in hospitals and the objectives of the Health Level 7 Reference Model (HL/-RIM) Version 2.02, HISA and German Frame architecture for Telematics in Health Care and will also adjust with other sources. We decided to take the Heidelberg requirements index for information processing in hospitals as a basis for describing the enterprise functions. The advantage to other comparable work is that these requirements are formulated independent of information processing tools or of information system architectures.

 

 

Results:

Creation and Evaluation of a reference model for the domain layer:      Our resulting reference model of Hospital Information Systems is hierarchically structured and consists in its actual state of one primary model with the central enterprise function “patient treatment” and the four cross-sectional enterprise functions “handling of patient records”, “scheduling and resource allocation”, “hospital management” and “research and education”.  This primary model is then subdivided into 21 submodels that describe the sub-functions according to their refinements. The elements of the domain layer of the reference model of a HIS were also adjusted with other resources like HISA [18] and the German Frame architecture for Telematics in Health Care. To check completeness and comprehensibility of the reference model, we organised workshops with employees of the strategic information management of hospitals, with care professionals, with employees of business consultancy for health care institutions and also with medical informaticians.

 

Practise and Evaluation of the 3LGM2-method and the respective tool:

Application projects:

- Modeling and evaluation of the HIS of the Psychiatric Hospital Hall (PKH Hall).

- Modeling and evaluation of the HIS of the Department of Dentistry and Oral Surgery at the Innsbruck University Hospital in Austria ZMK-LKI Innsbruck).

- Modeling and evaluation of the whole HIS of the Innsbruck Tyrolean Provincial Hospitals GmbH (TILAK).
